C6 with bolt on yoke

We are looking for a C6 transmission for a small block windsor with a bolt on yoke to build up. Did they come from the factory this way and in what vehicles or are people just building them out of parts, maybe from a big block C6? I have only seen them in big block motorhomes but they have a big parking brake attached.

Look at early 80's 4X trucks with divorced transfer cases. I believe some vans had them also.
